Peds type 1 COVID-19 study idea

Just this week I've admitted two kids with new onset T1D whose parents subjectively delayed presentation for care out of concern for coming to the hospital 'since that's where all the coronavirus is.' I wonder whether a delay in seeking care will change the presentation (more DKA?) or will home quarantines reveal to parents polyuria/polydipsia sooner than would have been recognized otherwise and result in less DKA at diagnosis?

    Evan,

That is interesting idea, and certainly an unintended, but plausible adverse consequence of the COVID-19 environment.  I am not physician, but lead the T1D Exchange and our Quality Improvement Collaborative, which does track DKA cases among a number of other useful data points monthly from our participating clinics (we have built interface to receive EMR data for all Type 1 patients at these clinics).  I will forward this idea to our head of Population Health and QI, Dr. Osagie Ebekozien for consideration.

In addition, below is info on a survey-based study  we are launching today to track cases of T1D and COVID-19.  I encourage you to read about it and consider joining.

The T1D Exchange QI Collaborative is excited to launch the T1D Healthcare Provider COVID 19/COVID 19 illness Registry/Surveillance Project today for pediatric and adult T1D providers https://t1dexchange.org/covid19/. This project will be co-lead by Dr. Mary Pat Gallagher (NYU Langone Health), Dr. Todd Alonso (Barbara Davis Center) and Dr. Osagie Ebekozien (T1D Exchange).

The first phase will help us answers some preliminary questions regarding COVID 19 or COVID 19 like illness (because of testing limitation) presentation and outcomes among patients with Type 1 diabetes using Provider reported relevant case reports.

The survey https://tinyurl.com/T1Dx-COVID19 is open to any T1D Healthcare Provider in the United States and can be completed in less than 15 minutes. No patient identifiable data will be collected as part of this initial study. We encourage each center to designate a Provider that will be responsible for entering the relevant cases in your clinic to avoid duplication.

You can find answers to some of FAQs, view central IRB protocols and approval https://trello.com/b/gix9FGc2/covid-19-study. The Lead P.Is will share aggregate data and insights periodically via different channels to the T1D community for real-time learning and quality improvements.
